Usually you have to go to Istituto Varelli (Fuorigrotta area) for the medicals the day before the interview.
It depends on what you plan to do: the train is very convenient and you can reach the Varelli by taxi when you are there.
U.S. consulate is within walking distance from Mergellina train station
I can't remember exactly the difference between the two "certificati" but if I'm not mistaken the "casellario giudiziale" include "certificato penale"and some other things....
Can't remember either the vaccination issue, but if something is missing they are going to complete the requirements during the medicals

Thanks Rangers, another question is, do I have to download form DS3026 and fill it or it will be done after the medicals?
