Vaccinations - List and costs, plus the visa cost
 | The list of required vaccinations has been updated, but keep in mind that this is what the US Consulate in Italy requires: if you live in another country requirements might be different; check with your Consulate and try to get all your vaccinations beforehand, it will probably be cheaper.
  | There is a vaccine administration fee to pay, and the amount is 15€ regardless of the number of vaccines.
   | TD (tetanus diphtheria): 10€
   | MMR (Measles-Mumps-Rubella): 20€
   | DPT (defter) Children: 7€
 | Vaccination list by age of applicant - the source is the US Embassy in Seoul, Korea.
PLEASE NOTE THAT THE FINAL DECISION BELONGS TO THE PHYSICIAN.
 | Cost of the physical examination:
Adults: 160€ (95€ for dependents of Military personnel) Persons under 15 years of age: 30€ (20€ for Military Personnel dependents).
The cost of the visa is $335 + $100 DV-Lottery surcharge; consider the $100 as a sort of 'you lucky ba$%$d' tax.
You will pay the fee for the medical examination in your local currency -at least in Europe- to the doctor before the examination; the $435 for the issuing of the visa shall be paid in US$ to the Consulate cashier the day of the interview.
